Application for Residency

The information on this application will be kept confidential. A non-refundable application fee of $50 per person is required at the time the application is submitted. An application must be completed for each individual applying.  The application must be fully completed and include all requested documentation (see last page of application) at time of admission.  If you have any questions while completing it, please do not hesitate to contact us.

Admittance to The Virginian will be contingent upon a satisfactory medical assessment and physical exam in addition to the attached financial statement. Please inform us of any health changes you experience after submitting your application.
